PLAN of Southeastern Minnesota
PLAN of Southeastern Minnesota is a unique project hosted through Arc Southeastern Minnesota that provides options to families concerned about the future lifetime care and quality of life of a loved one with a chronic disability and/or mental illness. PLAN provides services and supports when family members are not available or are unable to do so. These supports can relieve family members and close friends of overwhelming responsibilities.

- INDIVIDUALIZED PLANNING SERVICES:
PLAN of Southeastern Minnesota provides individualized planning services for persons with developmental or physical disabilities, mental illness and/or brain injury, as well as other disabilities. PLAN works together with individuals and families.
- PERSONAL SUPPORT PLAN:
A Personal Support Plan provides a comprehensive history of the family member's life and it serves a way for families to express their future wishes and concerns for a loved one with a developmental/physical disability, mental illness, brain injury or other disability.
- LIFE PLANNING PARENT/FAMILY WORKBOOK:
This workbook is designed for families to record pertinent information regarding their loved one.
- FEES:
Payment options are available for your convenience: cash, check, credit card & other.
- INFORMATIONAL SESSIONS and WORKSHOPS:
Informational sessions and workshops are available, if requested.
