Intake and Assessment
Personal Support Plan
Development |
This includes the PLAN Project Director or staff person to facilitate the planning process. A meeting with the individual, family members and support persons, as desired (The intake typically takes between 2 – 4 hours.) The PLAN Project Director or staff drafts the Personal Support Plan with the information from the intake and assessment meeting. The Plan is then sent to the family for approval. Modifications are made. An additional meeting is scheduled, if necessary. (The draft typically takes between 2-3 hours.) |

Personal Support Plan
Updates, Tracking and Recording & Renewal Fee |
Includes monitoring and maintaining services within the Personal Support Plan. The Plan is reviewed and updated annually (minimum). Minor updates and changes may be made at any time via telephone, e-mail or at a meeting. (There is no extra charge for minor updates or changes.) |

This is an additional service that is contracted for and monitored within the Personal Support Plan. Some examples may include: coordination of services, personal visits to your loved one, the conservator and other family members, as well as assistance with providers (including residential and day programs, meetings, governmental hearings, etc.) and assistance with completing forms and applications. |
