About planning for what I need and want . . . What do I need to know about planning my supports? You are the person who should be in charge of your supports. Participating in a planning process should lead to choices and supports that help you move toward your vision of the future. Basic information about planning, time commitment, who should be involved, etc. should be available from your county case manager, public health nurse or local advocacy organization. Who can help me plan for my personal supports? It is helpful to have someone assist you in deciding what you want to do with your life and what help or supports you may need to accomplish your plans. Your case manager or public health nurse can help you find someone to work with you and the people in your life to plan your supports. About the supports and services I need . . . Who can I hire to provide me with the supports I need and want? You should be able to choose who you want to provide the supports you need. Your case manager or public health nurse can give you information on formal provider agencies as well as information about hiring your own supports. Where can I find people to provide me with support? There are several options available for hiring people you know and trust to provide support. Your case manager or public health nurse can inform you of these options. |
